2. Protected Health Information

If your personal data includes health information, it is treated in accordance with the applicable Notice of Privacy Practices. Cognitive Diagnostic Solutions operates as a “business associate” when providing Services with affiliated healthcare providers. Protected health information is handled according to HIPAA and related laws.

3. Children Under 13 and Users Under 18

Our Site and Services are not intended for children under 13, and we do not knowingly collect information from children under 13. Users under 18 must have authority to consent to use our Site and Services; if they do not, they may not provide information. This Notice explains how Cognitive Diagnostic Solutions and affiliated providers (“we” or “our”) may use, disclose, and protect your health information, and your rights regarding that information. “Protected Health Information” (PHI) includes any data that can identify you and relates to your health, treatment, or payment.

Uses and Disclosures of PHI:

We may use PHI to provide and coordinate care, manage billing, improve services, conduct audits and compliance activities, and comply with legal requirements. De-identified information may be used for research or analytics. PHI will never be sold or commercially marketed.

Uses and Disclosures Without Your Authorization:

PHI may be disclosed when required by law, for public health, law enforcement, legal proceedings, research, national security, or for other authorized purposes.

Student Users:

If accessing Services via an educational agreement, PHI may be shared with University providers or officials as necessary to deliver care and services.

Uses and Disclosures Requiring Authorization:

Other uses require your consent. You may request an accounting of disclosures and receive a paper copy of this Notice at any time.

Breach Notification:

If a reportable breach of your unsecured PHI occurs, we will notify you within 60 days of discovery with details about the breach and your rights.

Complaints:

Complaints regarding PHI handling may be directed to our HIPAA Privacy Officer. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not result in retaliation.
